Create Patient /Add Patient

1. Click on Patient Management on the left panel.

2. Click on the Add Patient in the right window.

3. It will open Create new Patient Page.

4. Fill in all the necessary details of the new patient.

5. Click on Submit.

6. A new patient is now created.

For e.g. MRN (Medical Record Number) – It is the unique id for the patient.

First Name, LastName, Date of birth, Gender, Email, Mobile and Activity are some mandatory fields.

Patient Health Info

Select the ‘Chronic Condition’ for the Patient and the ‘Vital’ necessary for the patient.

On Selecting the ‘Vital’ their measuring scale appear below them. These scales are set when we check ‘Set Default’ however the scale can be adjusted manually if required.

Frequency field – how many times the patient wants to give reading for that vital. According to the frequency time patient will get a reminder notification for the next reading time.

Patient Monitoring Date:

Monitoring start date and end would be selected by healthcare provider-Doctor/Nurse/Admin of the hospital. When the Patient is willing to start using the services they can inform.

Monitoring Start date –It set once at the time of form filling henceforth cannot be changed

Monitoring end date -can be changed.

Provided Devices: If the patient wants to give their reading through a mobile app or the devices directly.

They choose accordingly from the option ‘Bluetooth’ or ‘Cellular’.

Care Giver Info: If the patient has a caretaker with them they can add their detail. It is not a mandatory field, up to patient confirmation they want to remove detail or not.

Payer: The user can select a patient who is Medicare type or commercial type. They have to add note to it as it is a mandatory field.

Assigned to: The Nurse and Doctor going to be in charge of the patient is going to be selected from the drop-down box.

Device Procured: If the devices are procured by Some provider, or through insurance, or patient has arranged for devices themselves.

Monitoring Status: On /Off.

On – Patient is active is monitored regularly as suggested by the doctor

Off – Patient is inactive and is not being observed by HealthArc provider.

If a patient wants to discontinue the services of HealthArc or patient observation is completed.

Acknowledgment check box:

Acknowledge is mandatory for each patient and nurse for billing.

After Nurse/doctor has given training to patient-related to devices or readings they check to acknowledge only for the first and last time. Until the Nurse/Admin/Doctor will do acknowledge then their billing code (99453) will be generated.

Click Submit Button to add the Patient and click Cancel to cancel the process.
