All Chronic Conditions
1. Click on Dashboard on the left panel.
2. Click on the All Chronic Conditions filter in the right window. 
3. Clicking on All Chronic Conditions the following options are displayed in the dropdown.
5. Click on Apply.
6. It will display records of the patients as per the selection made.
For example, If any patient's chronic condition is Diabetes, then that patient will be shown in the filtered patient list of Diabetes. Similarly, if any patient has both chronic conditions CHF and COPD then it will show in both the filtered patient list of CHF and COPD respectively.
