The various CPT codes are discussed below:

 



CPT 99453

Remote monitoring of physiologic parameter(s) (eg, weight, blood pressure, pulse oximetry, respiratory flow rate), initial; set-up, and patient education on the use of equipment

CPT 99454

Remote monitoring of physiologic parameter(s) (eg, weight, blood pressure, pulse oximetry, respiratory flow rate), initial; device(s) supply with daily recording(s) or programmed alert(s) transmission, each 30 days

CPT 99457

Remote physiologic monitoring treatment management services, clinical staff/physician/other qualified health care professional time in a calendar month requiring interactive communication with the patient/caregiver during the month; initial 20 minutes

CPT 99458

Remote physiologic monitoring treatment management services, clinical staff/physician/other qualified health care professional time in a calendar month requiring interactive communication with the patient/caregiver during the month; additional 20 minutes

99490

The code covers non face-to-face services provided to qualifying patients: those with two or more chronic conditions such as hypertension or diabetes. This 99490 code is generated for each patient per calendar month who receives a minimum of 20 minutes of care from clinical staff directly by a qualified healthcare professional.

G2058

G2058 is a code to capture additional care management time. Every additional 20 minutes is billed

 

  1. Code-99453: This code is automatically generated when the patient's monitoring starts.
     Follow the steps given below to generate this code:
  1. Click on the Edit Profile option in the Patient's profile.
  2. Scroll down to the bottom of the profile
  3. Put the Monitoring Status 'On' as below

 

 

  1. Code-99454: This code will only be generated if an active patient's readings are recorded on a minimum of 16 unique days in a month.
  2. Code-99457: This code is generated when Remote Patient Monitoring is done by the doctor or nurse for the initial 20 minutes in a calendar month. To add time to the patient's profile visit the patient's profile, make a video or voice call, and add notes related to the patient. Apart from this, RPM time can also be added manually by following the below steps:
  1. Click on the Dashboard from the Main Menu. The following screen will appear:
  2. Click on the timer which appears at the right-most corner of the Patient's record displaying the total time spent on a patient by the health care provider
  3. On clicking the above timer, the following screen appears

  4. Select the activity for which the note is being added from the drop-down next to Activity for

  5. Click on the Drop Down button under the 'Add Extra Time' option

4.  Code-99458: This code is generated for every additional 20 minutes spent for Remote Patient Monitoring in a calendar month, apart from the above first 20 minutes for code-99457. The procedure to log the additional time is the same as that for code-99457

5. Code-99490: This code will only be generated for those patients who have opted for both CCM and RPM. This code is generated when Chronic Care Management service is taken for the initial 20 minutes in a calendar month. For adding time for CCM, it is required to change activity from RPM to CCM. To add time to the portal for a patient, visit the Patient's Profile and make a video or voice call or add notes related to the patient. The CCM time can also be added manually through the following steps:

  1. Select the 'Add Notes' option for a particular patient from the dashboard.
  2. Click on the drop-down button under the 'Add Extra Time' option

6. Code-G2058:This code is generated for every additional 20 minutes spent for Chronic Care Management in a calendar month, apart from the above first 20 minutes for code-99490. The procedure to log the additional time is the same as that for code-99490.
